HCP is a program for children and youth with special health care needs and a community resource for families and health care providers in Weld County. HCP serves children from birth to age 21 who have or are at increased risk for having a chronic physical, developmental, behavioral, or emotional condition and who also require health and related services of a type or amount beyond that required by children and youth generally.
Our nurse-led teams work with children and youth with special health care needs and their families to connect them with medical, behavioral health, oral health, developmental, educational, and community resources in Weld County.

Eligibility
There are no financial or diagnostic criteria. Any child or youth with a special health care need may receive services. Any family can use these care coordination services for their child/youth at no cost, regardless of income or diagnosis. It is important to note HCP is not an insurance plan and is unable to pay for services.
