All applications are due at least 2 weeks prior to the event start date.

Applications

All temporary event applications must be submitted online. The Health Department has a public computer you may use if you don't have access to one. 

2022 Applications

Event Coordinator Application (for event coordinator/person or organization hosting the event)

Food Vendor License Renewal or No License Required (food truck, non-profit, cottage food, wholesale, sampling, etc.)

New Food Vendor Application (for first time vendors, vendors with major changes, or vendors who have not been licensed in Weld County in the last 3 years)

2023 Applications

Event Coordinator Application (for event coordinator/person or organization hosting the event)

Food Vendor License Renewal or No License Required (temporary food establishment or licensed food truck)

New Food Vendor Application (for first time vendors, vendors with major changes, or vendors who have not been licensed in Weld County in the last 3 years)

Non-profit Food Vendor Application

NEW FOR 2023 - Special Event Vendor Application (food vendors preparing or serving non-TCS foods at coordinated and uncoordinated events... kettle corn, roasted nuts, snow cones, etc.)
