Health Determinants Report and Assessment

Health Determinants and Assessment Report Health starts where we live, learn, work, and play. And we know that the condition in which we live and/or work can have an impact on our health. This report, A Look at Health and Its Determinants Across Weld County, a Community Health Assessment Report, provides an update on our population’s health overall and looks at the relationship between health and some of the determinants of health that influence residents’ health.

Health determinants are factors, upstream from personal health behaviors and genetics, that influence individual health status. Factors like income, housing, education, employment, sense of belonging (i.e. being connected to family, friends, and community), and our early years often are linked with difference in health outcomes and lifespan. Opportunities for better health increase when differences in these social factors are addressed.

What is Public Health?

Cover of the Health Department's What Is Public Health BrochurePublic health is the science of protecting and improving the health of people and their communities. Public health is population based, meaning it focuses on improving the health of the entire community rather than addressing individual health care needs. Prevention, people, and partnerships sum up the work of local public health.

Weld County established the second local health department in Colorado in 1938 and is the longest continuously operating local health department in the state. The Weld County Health Department achieved accreditation from the Public Health Accreditation Board in November 2016, making it one of eight agencies to earn this recognition to date in Colorado. The value of accreditation strengthens the Health Department to better serve Weld County residents through continuous quality improvement, partnerships, and accountability.

The Weld County Health Department is organized into four service divisions: Administration & Finance; Environmental Health Services; Health Education, Communication, and Planning; and Public Health Services.

View and download the Weld County Health Department Brochure to learn more about the programs and services offered by our four divisions.

10 Essentials of Public Health Services

The 10 Essentials of Public Health Infographic

The 10 Essential Public Health Services (EPHS) provide a framework for public health to protect and promote the health of all people in all communities. To achieve equity, the Essential Public Health Services actively promote policies, systems, and overall community conditions that enable optimal health for all and seek to remove systemic and structural barriers that have resulted in health inequities. Such barriers include poverty, racism, gender discrimination, ableism, and other forms of oppression. Everyone should have a fair and just opportunity to achieve optimal health and well-being.

Social Determinants of Health

Social determinants of health (SDOH) are defined as the conditions in which people are born, grow, live, work, and age. SDOH are shaped by the distribution of money, power, and resources throughout local communities, nations, and the world. Differences in these conditions lead to health inequities or the unfair and avoidable differences in health status seen within and between countries.
