Community Health Improvement Plan

Weld County’s Community Health Improvement Plan (CHIP) is an action plan to address key health issues. The community identified these priorities through a collaborative voting process. They include healthcare access, mental health, and chronic diseases. View the CHIP to learn more about the goals, objectives, and strategies for each health issue.

Regional Assessment Collaborative

WCDPHE and healthcare, other local public health agencies, behavioral health organizations, and Federally Qualified Health Centers in Weld and Larimer counties have come together to collaboratively partner on regional assessment and data activities.
