Children with Special Health Care Needs

HCP is a program for children and youth with special health care needs, serving as a resource for families and health care providers in Weld County. It supports children from birth to age 21 who have, or are at risk for, chronic physical, developmental, behavioral, or emotional conditions requiring more specialized care and services than typically needed.

Nurses and social workers connect families with the information and resources they need to manage their child’s health care.

The program’s goal is to empower families with the knowledge and skills needed to access services and support within the community, enabling them to coordinate their child's care with greater independence and confidence. HCP is dedicated to reducing barriers related to a child’s special health care needs and linking families to vital community programs.

Eligibility

  • Children and youth with special health care needs from birth to age 21 living in Weld County.
  • Your child may have a special health care need if they require health and related services more than that of other children generally.
  • Special health care needs can be:
    • Physical, emotional, or behavioral.
    • Diagnosed or undiagnosed.
    • Conditions that last a lifetime or end with treatment.
    • Easy to see in a child (visible health care needs) or not seen at all (invisible health care needs).
  • No income or proof of diagnoses required

Services

Weld County HCP helps you find:

  • Information, resources and connection to access services
  • Individualized Care Coordination
    • Each HCP Care Coordinator provides in-depth, one-on-one care coordination
    • An assessment is completed to evaluate the child’s areas of needs
    • A family-centered and assessment-driven shared plan of care is created to work on goals to minimize issues in the identified areas of need
    • Coaching and teaching to encourage parents to navigate challenging systems and to gain skills at being their child’s own advocate
  • Care Coordinators can help families build skills to:
  • find local services and supports.
  • plan and prepare for life transitions.
  • understand their child’s medical and behavioral health care needs.
  • access care and schedule appointments with providers.
  • coordinate communication across their child’s health care team.
  • connect to others who can address food, housing, and transportation needs.
