Meeting: July 18, 2025

The meeting was called to order at 8:16 a.m.
Presentation
Weld County DHS Case Management Agency Manager, Linda Medina, presented an overview of the Weld County Case Management Agency (CMA) which operates within the Home and Community Supports Division. She shared that the CMA provides supports to individuals from early childhood through the end-of-life and assists with transitions between services as needs change. Linda provided information about the many programs that they work with such as Medicaid, Long Term Care (LTC), Home and Community-Based Services (HCBS) Waivers, and State Funded Programs.
Introductions
Review of Minutes from April 18, 2025
Motion to approve the minutes from April 18th, 2025, was made by Gillian Smith and seconded by Adam Turk. The minutes were unanimously approved.
2024 Core Services Family Preservation/Placement Alternative Commission Report
Ryan Traylor from the Weld County Department of Human Services Child Welfare Division presented an overview of the 2024 Core Services Family Preservation/Placement Alternative Commission Report.
Community Services Block Grant (CSBG)
Expenditure Report – Julie Witkowski presented the CSBG Expenditure Report as of June 2025.
2026 Budget Review – Julie Witkowski presented the proposed CSBG Budget for 2026. Motion to approve the CSBG Budget for 2026 (Year 3) was made by Enita Kearns-Hout and seconded by Stanley Graffis. Motion passed unanimously.
Customer Satisfaction Survey Results
Julie Witkowski reported on the Satisfaction Survey results regarding the Trust Based Relational Intervention (TBRI) training that is offered to Foster/Adoptive/Kinship caregiver families and professionals. She also went over survey results for their support groups for Foster/Adoptive/Kinship caregiver families that are ran by the Prevention Team.
Strategic Plan Update
Kyle Ewertz presented the DHS Strategic Plan Guiding Principles that have been developed for the next three (3) years.
Contracted Services Update
Lesley Cobb provided an update regarding DHS contracted services for the 2nd quarter of 2025.
Lesley Cobb presented an overview of the Request for Application (RFA) which is a new method that DHS is using to seek providers for various Child Welfare professional services as well as other services needed within the department.
Division Audits/Evaluations/Reports
Ed Webster, Adult Protection Supervisor, reported on the Adult Protection Services (APS) Audit that was completed in March for the performance period December 26, 2024, to February 13, 2025.
Jill Colavolpe talked about the recently completed yearlong grant program for Long Term Care. This grant was award for a Long Term Care Case Management position within our Case Management Agency (CMA).
