Breaking Down Silos for Better Outcomes

Three children, three parents, and multiple placements created a highly complex case requiring exceptional flexibility and collaboration. Guided by our mission to engage and partner with the community to improve safety, health, and well-being, Child Welfare employees worked closely with the Family Resource’s Wraparound program, the Clinical Care team, and external partners to keep the children’s needs at the center.

The Clinical Care team, which supports child welfare by working directly with the providers who serve our families, youth, and children, played a critical role. They partnered with caseworkers to determine the best provider fit for each child and parent, ensuring services aligned with each family’s unique needs. They also sought appropriate placement options for youth with complex needs and drew on their strong relationships with facilities and service providers to ensure the highest level of care and support. Their involvement ensured that service provision remained purposeful, coordinated, and responsive throughout the case.

Together, the team navigated funding, housing, and crisis interventions, demonstrating our vision of connecting people to resources that help them thrive. The Wraparound team hosted frequent meetings and responded to crises, while Clinical Care provided targeted support, guidance, and coping strategies for both youth and caregivers. External partners, including law enforcement and Court Appointed Special Advocates (CASA), ensured safety and continuity.
