A single mother was referred to the Housing Stability Case Management program by Lighthouse, within Weld County Department of Human Services, due to unstable living conditions. While the family was safe and housed, the mother felt trapped without a clear path forward. Together with her case manager, she created a Housing Stability Plan with specific goals and actionable steps to provide direction. The mother’s only income was temporary alimony and child support. Having been a stay-at-home mom, she was unsure about her employment options. The case manager referred her to Employment Services of Weld County (ESWC), while continuing to help her apply for local subsidized housing waitlists. With guidance and support, the mother gained confidence and made progress toward a more stable living situation. She focused on her coursework through ESWC, and her name moved up on the housing waitlists. Her confidence grew through her success in phlebotomy training, and she was ready to tackle her housing challenges. Though nearing the top of the subsidized housing list, no move-in date had been confirmed. The client continued pursuing entry-level positions in her new field while securing temporary employment with Greeley-Evans School District 6. Encouraged to consider non-subsidized housing options, which had previously seemed out of reach, she applied based on a potential budget. Today, the client has signed a one-year lease and her family started the new year in a safe, stable home. The program provided Emergency Rental Assistance II funds for move-in costs, and the client is confident in her ability to manage the rent independently. This case showcases the successful collaboration of multiple departments in supporting families in the community.
