Foster Youth in Transition Services helps young people ages 10 to 21, including foster youth who are getting ready to live on their own. The program supports kids and teens who are having problems at home or in their community. Sometimes, a judge asks for help through a court order. The goal is to help youth get along better with their families, their communities, and society.

How to Apply

If you think you or someone you know might need help, you can contact the Youth Services department directly. They will talk with you and your family to see what kind of help is needed and if you qualify.

Who Can Apply

Youth ages 10 to 21 may qualify, especially if:

  • They are having trouble at home or in the community
  • They are in foster care and getting ready to live on their own
  • A court has asked for help because of behavior or safety concerns

Services Offered

  • Help solving problems between youth, families, and the community
  • Support to make families stronger and safer
  • Plans to help youth become more independent
  • Meetings with youth and families to figure out what help is needed
  • Therapy at home, in the community, or in special care centers
  • Help following court orders and sharing plans with the court
  • Referrals to other helpful programs in the community
