Family Resource Division

Family of 4 speaking with a caseworker The Family Resource Division supports families of Weld County by offering a variety of services.  All programs within the Family Resource Division are free, voluntary and can be accessed by self-referral. 

The Family Resource Division includes the following programs:

Outreach

The Outreach team helps Weld County families with public assistance benefits and services provided by the Department.  Service Coordinators explain guidelines, eligibility requirements and assist with the application process. They offer these services at all Weld County Department of Human Services locations and throughout the community (see below for current schedule of outreach sites).

Service Coordinators are connected with and refer clients to other community programs.

The Outreach team helps individuals and families request emergency kinship TANF supportive services. Upon availability of funds, one-time funding can be provided to assist families in crisis in achieving or maintaining household stability.  Funds can also be provided to support the safety of the children in their care.  Supportive Services are NOT guaranteed and are available only to families who are in extreme financial distress and meet eligibility requirements.

Post Adoption Support (Family Forever)

The Post Adoption Support Services program strives to provide families a safe place to discuss their struggles, ask for and receive support, and obtain the necessary resources needed to strengthen their adoptive family.

Family Forever - Weld county permanency support program The primary goal of the Post Adoption Support Program is to preserve permanency for children who have been adopted and to ensure the well-being of all members of the household.  In order to do so, it is essential that all families have access to trauma-informed resources.

Adoptive families may struggle with: changing developmental needs, unresolved past trauma experiences, changes in family dynamics and/or changes in educational/mental health or medical needs.

Eligibility: Families who have adopted children (any type of adoption) and live in Weld County, and families who have adopted from the Weld County Child Welfare system and live elsewhere.

Adoption Support and Resource Coordinators provide outreach, consultation, referrals to community resources and case management services upon request.

Adult hands holding child's hands holding a heart Participation in the Post Adoption Support Program is voluntary.  Adoptive families are encouraged to reach out to the Adoption Support and Resource Coordinators at any time, even if they initially decline involvement.

Services Include:

  • In home, email or telephone consultation and case management
  • Resource lending library
  • Training opportunities
  • Sharing information on available community resources

Lighthouse Family Navigation and Support Team

The Lighthouse Family Navigation and Support Team offers free professional case management to families who live in Weld County. Case Managers partner with families to identify strengths and needs. Then Case Managers will research and connect families to programs and services within the community which help in the achievement of their goals.

The mission of Lighthouse is to prevent family disruption and to assist families in maintaining safety, permanency and well-being through empowerment and a strengths-based approach.

Eligibility: All families who reside in Weld County with a child or children between the ages of 0 months to 18 years in the home, as long as they are not actively involved in a Dependency and Neglect Child Welfare Case. Services are also available to families with children over the age of 18, as long as the child is a full-time student and are expected to complete their education before age 19.

Participation in Lighthouse is voluntary. Families are encouraged to reach out for services at any time, even if they initially decline involvement.

Case managers are also available to assist families struggling with homelessness with navigation and accessing affordable housing programs and services. 

The Lighthouse Family Navigation and Support Team is offered in partnership with Weld County Department of Public Health and Environment.

Access to bilingual case management and interpreters available.

Housing Stability Case Management

The Housing Stability team works with Weld County residents who are experiencing housing instability or are at risk of homelessness. Our team provides strengths-based case management: working to help customers identify barriers, provide referrals for appropriate services and supports and ultimately aim to help individuals and families remain housed or become stably housed.

Services include, but are not limited to:

  • Partnering with the household to create a Housing Stabilization Plan
  • Facilitating mediation between landlords and tenants
  • Implementing eviction prevention services
  • Assisting with locating housing resources
  • Providing referrals to resources which can help further housing stabilization goals
  • Offering support around budgeting, life skills, tenancy skills, and effective communication

NOTE: Limited emergency Rental and/or Utility assistance may be provided to support the stability of households, however, financial assistance is NOT guaranteed and is available only to households who meet eligibility requirements and who have not already exhausted Emergency Rental Assistance (ERA) funding limitations. Prioritization for funding and case management services will be given to households with income 50% or less Area Median income.

In order to be considered for funding, the following criteria must be met:

  1. One or more individual(s) within the household has qualified for unemployment benefits or experienced a reduction in household income, incurred significant costs, or experienced other financial hardship during or due, directly, or indirectly, to COVID-19; and  
  2. One or more individual(s) within the household can demonstrate a risk of experiencing homelessness or housing instability; and
  3. Assistance will allow the case management customer to remain stably housed or to obtain new housing; and
  4. The household is a low-income family as defined in section 3(b) of the United States

Housing Act of 1937 and falls within one of the income categories outlined below.

Foster, Adoption, and Kinship Support Group

Join us to connect with other foster parents, kinship caregivers, adoptive parents and child care providers to share parenting strategies, successes and struggles with those who can relate! We focus on support around parenting and caregiving issues using a nationally recognized, research-based framework for keeping children safe and families strong. Groups are held from 6 - 8 p.m. at the Weld County Department of Human Services located at 315 N. 11th Ave, Bldg. A, Greeley, CO 80634. Dinner and child care is provided!

Trust-Based Relational Intervention®

Trust-Based Relational Intervention® (TBRI®) is an attachment focused, evidence-based parenting and intervention model. It is a trauma-informed intervention that is specifically designed for parents and caregivers of children who come from ‘hard places,’ such as maltreatment, abuse, neglect, multiple home placements, and violence, but is an approach that can be used by parents and caregivers with all children.
