Keeping people safe and well is our priority. If something serious happens to someone receiving case management support, please report it right away. These serious events, or "critical incidents," could include injuries, abuse, neglect, medical emergencies, or anything that puts a person at risk.

Why It’s Important to Report By reporting incidents quickly, you help us:

  • Protect people from harm and make sure they get the help they need
  • Make any changes to improve care and safety
  • Keep case management services at a high standard

What to Report Please report incidents like:

  • Serious injuries or medical emergencies
  • Any kind of mistreatment, abuse, neglect, or exploitation (MANE)
  • Unexpected trips to the hospital or big changes in health
  • Dangerous behaviors or situations affecting others' safety or property
  • Events involving the police or law enforcement
  • Medication mismanagement
  • Unsafe housing or removal from housing
  • Death

How to Report Reporting is simple and private. You can contact your case manager or fill out a secure report online. Each report is carefully reviewed so we can take the right steps to keep everyone safe.
