AAA Community Partners

To better serve residents, Weld County AAA contracts with community partners to deliver specific services, which are listed below by topic. These services can also be found in the main resource guide.

Case Management

Case management services for at-risk older adults age 60 and over to help them remain safe and independent. Services may include advocacy, assistance with applications and forms, home visits, information, referral and more. For more information, contact Catholic Charities at (970) 353-6433.

Hispanic Outreach

Bilingual Case Managers assist Spanish-speaking older adults by connecting them to community resources. For more information, contact Catholic Charities Hispanic Outreach in Fort Lupton at (303) 857-2844.

Home Delivered Meals

Specially prepared, nutritious meals are delivered to the home five days per week. For more information, contact Meals on Wheels in Greeley at (970) 353-9738.

Legal Services

Legal services for Weld County residents age 60 and over to assist with public assistance benefits, preparation of living wills and powers of attorney, landlord or rent issues, senior housing and other personal rights issues. Peer Counseling

Counseling services help older adults cope with the emotional stresses related to aging. The service is provided in the older adult’s home by trained counselors who are also older adults. For more information, contact North Range Behavioral Health at (970) 347-2420.

Transportation

Volunteers provide transportation for appointments, grocery shopping, trips to senior centers and more for older adults in need. For more information, contact 60+ Ride at (970) 352-9348.
