Open or Close a Child Support Case

How to Apply

Apply for child support online with the Application for Child Support form, or pick up a paper application at any Human Services location.

Who can apply?

• Mother of child
• Father of child
• Caregiver

Application Requirements

• Child(ren) are not emancipated
• Name, Address and social security number
• Name of other parent (if known)
• Name, birthdate, gender, place of birth, and social security number of child(ren)
• Applicants signature (hand written or e-signature)
• Social Security Number (if applicable)

I want to close my case

To close your child support case, contact our office at (970) 400-6499 or email childsupport@weld.gov.

I want to reopen my case

To reopen a case, you must submit a new application.
