Family Resource

The Outreach team helps Weld County families with public assistance benefits and services provided by the Department.  Service Coordinators explain guidelines, eligibility requirements and assist with the application process. They offer these services at all Weld County Department of Human Services locations and throughout the community (see below for current schedule of outreach sites).

Service Coordinators are connected with and refer clients to other community programs.

The Outreach team helps individuals and families request emergency kinship TANF supportive services. Upon availability of funds, one-time funding can be provided to assist families in crisis in achieving or maintaining household stability.  Funds can also be provided to support the safety of the children in their care.  Supportive Services are NOT guaranteed and are available only to families who are in extreme financial distress and meet eligibility requirements.

The Post Adoption Support Services program strives to provide families a safe place to discuss their struggles, ask for and receive support, and obtain the necessary resources needed to strengthen their adoptive family.

The primary goal of the Post Adoption Support Program is to preserve permanency for children who have been adopted and to ensure the well-being of all members of the household.  In order to do so, it is essential that all families have access to trauma-informed resources.

Adoptive families may struggle with: changing developmental needs, unresolved past trauma experiences, changes in family dynamics and/or changes in educational/mental health or medical needs.

Eligibility: Families who have adopted children (any type of adoption) and live in Weld County, and families who have adopted from the Weld County Child Welfare system and live elsewhere.

Adoption Support and Resource Coordinators provide outreach, consultation, referrals to community resources and case management services upon request.

Participation in the Post Adoption Support Program is voluntary.  Adoptive families are encouraged to reach out to the Adoption Support and Resource Coordinators at any time, even if they initially decline involvement.

Services Include:

• In home, email or telephone consultation and case management
• Resource lending library
• Training opportunities
• Sharing information on available community resources

The Lighthouse Family Navigation and Support Team offers free professional case management to families who live in Weld County. Case Managers partner with families to identify strengths and needs. Then Case Managers will research and connect families to programs and services within the community which help in the achievement of their goals.

The mission of Lighthouse is to prevent family disruption and to assist families in maintaining safety, permanency and well-being through empowerment and a strengths-based approach.

Eligibility: All families who reside in Weld County with a child or children between the ages of 0 months to 18 years in the home, as long as they are not actively involved in a Dependency and Neglect Child Welfare Case. Services are also available to families with children over the age of 18, as long as the child is a full-time student and are expected to complete their education before age 19.

Referrals can come from families themselves, the community, as well as, other Divisions within the Weld County Department of Human Services. 

Participation in Lighthouse is voluntary. Families are encouraged to reach out for services at any time, even if they initially decline involvement.

Case managers are also available to assist families struggling with homelessness with navigation and accessing affordable housing programs and services. 

The Lighthouse Family Navigation and Support Team is offered in partnership with Weld County Department of Public Health and Environment.

The Housing Stability team works with Weld County residents who are experiencing housing instability or are at risk of homelessness. Our team provides strengths-based case management: working to help customers identify barriers, provide referrals for appropriate services and supports and ultimately aim to help individuals and families remain housed or become stably housed.

Services include, but are not limited to:

• Partnering with the household to create a Housing Stabilization Plan
• Facilitating mediation between landlords and tenants
• Assisting with locating housing resources
• Providing referrals to resources which can help further housing stabilization goals
• Offering support around budgeting, life skills, tenancy skills, and effective communication
