The Food Assistance Program provides a basic level of nutritional well being for eligible low-income families and individuals.

The Application Process:

To start the application process and establish your application date for the Food Assistance Program, you must fill out the application and return it to a local food assistance office.

How Do I Apply?

There are several ways to get an application form. 

Access the PEAK website, or

Download and Print an Application for Public Assistance, or

Call our local office and we will mail you an application, or

Pick one up at a local Human Services office

Fill out the application as completely as you can. Be sure that your name and address are listed and that you have signed the application. Let us know if you need help completing the application and a worker will assist you. 

Get the application to your local Human Services office as quickly as possible in person, by mail, by email BenefitDocs@weld.gov, or by fax (970) 346-7669

Your Interview:

You will be contacted by our office within seven (7) days from the date you submitted your application to complete your interview. If you don't hear from us, please contact us at (970) 400-6013 to schedule your appointment. Most interviews will be completed by phone, there is an option to complete it in person - depending on your situation.

Once your interview is completed and all required verification received, you're application will be processed and you will be notified by mail of your eligibility. If you have questions regarding your eligibility, call us at (970) 400-6012 to speak to someone.

You will receive a notice in the mail about your eligibility.

Not Eligible:

If you are not eligible for food benefits, and you feel you have been denied in error, you may request a local County Conference or a State Fair Hearing. 

Information regarding your rights is included in the notice mailed to you.
