About the Survey

Since 2007, the Weld Community Health Survey has been conducted every three years by Weld County Department of Public Health and Environment. In 2025, a geographically stratified random sample of about 20,000 households were contacted by mail to complete the survey either online or on paper. A total of 2,802 adult residents responded, resulting in a 15% response rate. The data were weighted by several demographic factors using U.S. Census Bureau estimates so that the results are generalizable to the entire county population. For more information on the survey methods, email he-healthdata@weld.gov for a methodology report.

The Weld Community Health Survey helps public health officials, municipal governments, health care providers, and local nonprofit organizations better understand residents’ health needs and use that information to help shape services, programs, and policies. The survey covers a wide range of topics, including access to physical and mental health care, insurance, health conditions and behaviors, preventive screenings, environmental issues, community concerns, and health-related social and demographic factors. The 2025 survey also explored issues such as loneliness, social isolation, and housing costs.

We’re excited to invite you to a special webinar where Weld County Department of Public Health and Environment (WCDPHE) staff will share highlights from the newly released 2025 Weld Community Health Survey (CHS) - the most comprehensive source of health information about Weld County residents.

Conducted every three years by WCDPHE’s team of data analysts, the CHS offers a deep look at the health of our community highlighting lived experiences, needs, and priorities. This local data can help provide insight and guide municipal governments, health care organizations, community partners, and many others who play a role in shaping health and well‑being in Weld County.

During the webinar, WCDPHE staff will walk through key findings across a wide range of topics, including chronic disease outcomes, behavioral health, access to care, affordability, and more. It’s a great opportunity to learn what’s changing, what’s emerging, and what matters most to residents.
