Immunizations

The Weld County Health Department's Immunization Core Program provides education about communicable diseases, health risk reduction, and vaccine-preventable diseases for adults and children to Weld County residents, physician offices, schools, daycare facilities, long-term care facilities, and local businesses within the community. Routine and recommended immunizations for both children and adults are available in the clinic, occasionally during local community events and during outbreaks and epidemics.

Immunizations For Children

The Weld County Health Department provides routine immunizations for children, including immunizations required by Colorado School Law.(PDF, 642KB) Immunizations are available for children 0-18 years of age through the Vaccine for Children program.

Travel Immunizations

The Travel Immunization Program Serves Northern Colorado, providing an in-depth view of the destination country’s health, safety, and security for those who will visit a developing country. Visits include a nurse’s review of the client’s health history, provision of appropriate pre-travel care, immunizations, and prescriptions for prevention of travel-related illnesses. Post-travel care includes discussion of common medical complaints in the returned travelers.

The Travel Clinic cannot provide travel information over the telephone. Please visit the Centers for Disease Control and Prevention (CDC) Travelers Health website for information related to a specific country.

Plan to visit the Travel Clinic 6-8 weeks before your departure.

EFFECTIVE IMMEDIATELY:  Weld County Public Health will no longer submit insurance claims for Travel Vaccines. Payment in full is due at time of service. All travelers will be required to pay the following fees:

  • Consultation Fee – This fee is due at time of service, per person, even if you don’t receive any vaccines and is non-refundable
  • Vaccine Fee
  • Admin Fee per Vaccine
  • If other non-travel vaccines are recommended and you choose to get them during your travel appointment, we will not submit it to insurance. Payment in full is due at time of service.

Appointments are strongly encouraged, especially for travel and I-693 visits. Walk-ins are accepted as available for routine immunizations.

Greeley Clinic hours:

Monday - Friday
8:00 a.m. - noon and 1:00 p.m. - 5:00 p.m.*

Fort Lupton Clinic hours:

Wednesdays only
8:00 a.m.-12:00 p.m. and 1:00 p.m. - 4:00 p.m.

If you are unable to make your appointment, please call to cancel so it is available for someone else.

Children younger than age 18 must be accompanied by a parent or a person who has authority to give consent.

Pricing and Payment

Full payment is requested at time of service. We accept Medicaid and CHP. Vaccine costs vary depending on insurance and patient age, and prices are subject to change. No child is turned away for routine vaccines due to the inability to pay. All travel immunizations must be paid for in full.
