Navigation Services Program

We are a community navigation team that provides information, education and referrals to resources throughout Colorado.

What We Do

We are a voluntary case management program offered at no cost to people living in Weld County. We guide people through life's challenges by offering helpful information, resources, and encouragement. We provide help finding:

  • Access to healthcare
  • Basic needs including food, clothing and transportation
  • PrEP (Pre-Exposure Prophylaxis for HIV prevention)
  • ADAP (AIDS Drug Assistance Program)
  • Connection to other community agencies and programs

Who We Help

We can work with any person living in Weld County. We offer bilingual case managers and interpreters to make sure every family feels heard and supported.
