Health First Colorado is Colorado’s Medicaid program. It gives free or low-cost health care to people who qualify. This includes children, pregnant women, parents, caregivers, adults, and people with disabilities.

When you apply for Health First Colorado, you are also applying for Child Health Plan Plus (CHP+). You only need to fill out one application for you or your family.

Who is Eligible?

You may be eligible if you meet one of these requirements:

  • Children ages 0-18 whose household income does not exceed 142% Federal Poverty Level (FPL)
  • Pregnant Women whose household income does not exceed 195% FPL
  • Parents and Caretaker Relatives (applicant must have a dependent child) whose household income does not exceed 68% FPL
  • Adults without dependent children whose household income does not exceed 133% FPL
  • You can also use the pre-screening tool on the Colorado PEAK website to see if you quality.

Programs for Children

Colorado has health programs that help children and teens get the care they need. These programs can help with:

  • Doctor visits, shots, and checkups
  • Dental and eye care
  • Mental health support
  • Prescriptions and hospital visits

Some programs also help children who:

  • Have serious health problems
  • Need care at home instead of in a hospital
  • Have disabilities and need extra support
  • Need help with daily activities or behavior

Programs for Pregnant People

If you are pregnant, you may be able to get free or low-cost health care through Health First Colorado (Medicaid). This program gives full health coverage to people who qualify to help you stay healthy during pregnancy and after your baby is born.

What's Covered:

  • Doctor visits
  • Prenatal care
  • Labor and delivery
  • Mental health and substance use care
  • Dental services

Programs for Parents and Caretakers

If you’re a parent or take care of a child, you may qualify for free or low-cost health care through Health First Colorado (Medicaid).

What’s Covered:

  • Mental Health & Substance Use Help: Free therapy and treatment services.
  • Cancer Care: Help for women ages 40–64 with breast or cervical cancer.
  • Dental Care: Free checkups, cleanings, fillings, and more.
  • Child & Teen Checkups (EPSDT): Regular health care for kids under 21.

Programs for Adults

If you’re an adult living in Colorado, you may qualify for free or low-cost health care through Health First Colorado (Medicaid) or other state programs.

What’s Covered:

  • Mental Health & Substance Use Help: Free therapy and addiction treatment.
  • Cancer Care: Help for women ages 40–64 with breast or cervical cancer.
  • Senior Dental Program: Discounted dental care for adults 60+ who don’t have dental insurance and don’t qualify for Medicaid dental benefits.

Long-Term Care Services and Supports (LTSS) Programs

LTSS helps older adults and people with disabilities live safely and independently at home or in their community. These services are part of Health First Colorado (Medicaid).

What’s Included:

  • Home and Community-Based Services (HCBS): Help with daily tasks like bathing, dressing, cleaning, and getting around.
  • Support for Caregivers: Help for people who care for loved ones at home.
  • Other LTSS Programs: If HCBS isn’t the right fit, there are other options.

A Case Management Agency (CMA) can help you find the right program and connect you with local resources.

Weld County Public Health Services

Weld County Public Health helps people stay healthy by offering low-cost or free health services. They teach people about staying well and provide care in clinics and the community.

Services include:

  • Cancer screenings for women
  • Shots (immunizations) for kids and adults
  • Family planning and birth control
  • Help for kids with special health needs
  • Nurse visits for first-time moms
  • Testing and treatment for STIs and HIV
  • Care for people with tuberculosis (TB)
  • Social work support and referrals

Their goal is to make sure everyone in the community can get good, affordable health care.
