Family Support Council (FSC)

Weld County CMA helps families who have a member with a developmental disability by reimbursing costs for essential supports like medical care, therapies, assistive technology, transportation, respite, and home or vehicle modifications.

The FSSP Council advises on program funding and welcomes community members, families, and professionals to join.

Weld County Case Management Agency Family Support Council By-Laws

Council Meetings: Family Support Council meetings shall be scheduled at regular times and open to the public. A ten-minute open forum shall be accorded at the beginning of each meeting for the purpose of public and constituency input.

Voting Quorum: For the purpose of voting, a quorum shall consist of 51% of the Family Support Council; the majority shall be parents and/or consumers.

Council Recruitment: FSSP eligible families, professionals, and individuals from the community who have expressed an interest in serving on the Council shall be recruited as members of the Family Support Council. The prospective FSC member will complete an application and information form, and if necessary, a waiting list for the FSC membership will be maintained by CMA staff. Vacancies on the Council will be filled with the priority given to eligible families.

Family Support Council Terms: Council member tenure shall be for three consecutive years with the option of re-electing a member to serve another term. A member may be reappointed to the Council after the completion of two terms if he or she has not served the Council for at least one year.

Council Chairperson: The Chairperson of the council shall be a family member of an individual with an Intellectual or Developmental Disability or Delay.

Procedure: The Chairperson of the Family Support Council shall be elected by a majority vote of a Council quorum.

Responsibilities

Purpose:

To provide guidance and assistance to the Case Management Agency (CMA) in the development of the Family Support Services Program (FSSP) while focusing on broad issues which impact families.

Guidance:

• Make recommendations to the CMA for overall policy and procedures of FSSP including how families are prioritized and found eligible.
• Be aware of and make referrals to local community resources available to assist families.
• Recommend budget parameters for the program, specific to the allocation of funds.
• Provide recommendations on defining how an emergency fund is implemented. The policy must include a definition of a short-term crisis or emergency and the maximum amount of funds a family may receive per event and/or year.
• Provide recommendations on defining the “other” service category.
• Provide recommendations on gathering feedback from families enrolled
• Identify areas of training needed for Council members and families enrolled.
• Review and sign the annual FSSP report written by the FSSP Program Supervisor.

Monitor:

• The implementation of the overall services provided in the defined service area.
• The effectiveness of the public awareness strategies
• The effectiveness of the services and supports provided in meeting the intent of the legislation.
• Family satisfaction regarding the program.
• The diversity of the families accessing FSSP compared to the demographics of the area.
• The financial reports of expenditures and allocations to families.

Dispute Resolution: In the event that the FSC agrees that there is a conflict or dispute regarding decisions of the FSSP, there is a process that will be followed. Resolutions will begin with the FSSP Program Supervisor and will end when the FSC accepts the outcome or until the dispute reaches the Department of Health Care Policy & Financing (HCPF). The FSC and CMA will honor and adhere to the decision reached through mediation with the DIDD. The FSC Chair will provide a written letter to the CMA and HCPF.
