The Community Health Division (CH) of the Health Department is empowering communities to improve health through data-driven education and skills. By using population-based strategies to reduce death and disability in Weld County, CH incorporates education, prevention, and data collection and analysis in coordination with partner agencies to improve quality of life in Weld County. CH also manages public health accreditation and health-related public information, including risk communication. Programs in the CH Division include:

Childhood Obesity Prevention

The Childhood Obesity Prevention program promotes breastfeeding, physical activity, and healthy eating in early childhood centers.

Communities That Care

CTC is a community-driven process that prevents youth substance use, delinquency, and violence by selecting and implementing effective programs and policies.

RELATE

This program focuses on equipping adolescents with the tools they need to make healthy, informed decisions related to relationships.

Walk Audits

A walk audit is a tool to help communities participate in making healthy, safe, and walkable streets.

Breastfeeding Support

The Health Department's Breastfeeding Support Program works to provide support for new mothers at their workplaces, child care providers, and in public venues.

Healthy Plate

Helping individuals and families with limited food budgets learn to shop for and cook healthy meals by teaching essential skills such as food/knife safety, label reading, meal planning and cost-saving strategies.

Heart of Weld

The Weld County Health Department offers the Heart of Weld, a National Diabetes Prevention Program designed for individuals who are prediabetic or are at high risk for developing prediabetes/diabetes.

Strengthening Families program

The Strengthening Families Program is a seven-week, evidence-based program designed to strengthen family relationships, improve communication, and support healthy youth development, ultimately reducing substance use and enhancing academic and family outcomes.

Substance Use Disorder Education and Prevention

This program supports education, communication, and prevention strategies for substance use disorder with a variety of partners in Weld County.

Tobacco Education and Prevention Program

The Weld County Tobacco Education and Prevention Program works to create a tobacco-free environment through education, prevention, and cessation services.

Weld County Regional Opioid Council

Using funds from a national settlement that included Colorado, the council aims to address 3 key areas: opioid use prevention, treatment, and education.

Well Body Program

This program is a six-week wellness class designed to support individuals in building sustainable, health-enhancing habits.
